The entry form
What you collect from entrants, and the checkbox that decides whether an answer ever shows up in your stats.

Name, email, mobile, business name and the tribe are collected as standard. The Entry Form Fields tab is for the extra questions you want to ask.
Adding a question
Add Field, then set the Item Name — this is the label the entrant sees and the column heading in your export, so write it as a question a person would understand. Choose the Type: Text or File Upload.
The Chart checkbox
Tick it for anything with a small set of repeated answers — which product they are interested in, how they heard about you, which branch they visit. Leave it off for free text like "tell us why in 25 words".
Ask for less than you want
Every extra question costs you entries. The whole point of a competition is the contact record, and you already get name, email, phone and tribe without asking anything else. Two or three extra questions is generous; six is a form people abandon.
Answers are counted loosely
For charted fields, an answer containing commas or line breaks is split and each part counted separately. That is helpful for a "tick all that apply" style question and misleading for free text — another reason to keep Chart for short, repeated answers.
