Asking recipients a question and collecting the answers: building a form, its fields, and where responses go.

Last updated September 2, 2026

A feedback form is a short page you can send people to from a campaign — a survey, a preference question, a reason for leaving. Answers come back against the person who gave them.

Building one

SettingWhat it is
Page heading and messageWhat the recipient sees at the top. Say why you are asking — response rates roughly double when people know.
FieldsThe questions. Short text, long text, or a list of options to choose from.
Thank you messageWhat they see after submitting.
Redirect URLOptional. Send them somewhere afterwards instead of showing the thank you message.

Writing questions people answer

  • Three questions, not ten. Every extra field loses responses.
  • Offer choices where you can. A list is one tap; a text box is a small essay.
  • Keep long text for the last question, and make it optional — the people who care will write, and the rest still submit.
  • Ask what you will act on. Collecting an opinion you have no intention of using annoys the people who gave it.

Reading responses

Responses are listed against the form. Because they arrive attached to the person, a strong answer is a lead: somebody who has just told you what they want is worth a call.
