Franchise provider becomes early adopter of federal GUIDE Model, bringing Medicare-funded respite care to millions of families nationwide

  • Over 6.7 million Americans affected by dementia in 2023.
  • GUIDE model makes dementia care more affordable and accessible.
  • Families gain access to personalized care and trained caregivers.

ComForCare has emerged as a key player in the federal government's ambitious effort to transform dementia care through its participation in the Centers for Medicare & Medicaid Services' GUIDE Model. The franchised home care provider announced its role as a subcontractor in the newly launched program, which makes Medicare-funded dementia services accessible to families across the United States for the first time. The timing coincides strategically with National Dementia Awareness Month, highlighting the urgent need for comprehensive support systems as dementia cases continue their relentless climb.

According to CMS data, more than 6.7 million Americans lived with dementia in 2023, with projections indicating this figure will surge to 14 million by 2060. Despite these staggering numbers, individuals with dementia often receive inadequate care while families struggle without essential support resources, leading to increased hospitalizations and emergency room visits. The GUIDE Benefit represents a paradigm shift in dementia care delivery, allowing individuals to remain safely in their homes longer while providing critical relief to overwhelmed caregivers through comprehensive services covered under Medicare Parts A and B.

"July 2024 marked a turning point for dementia care, when CMS named 330 GUIDE organizations, allowing ComForCare to become a subcontractor, which allows our franchise locations to bring awareness of the program to our communities and provide the Medicare-funded respite services to families nationwide," explained Stephanie Wierzbicka, Director of Strategic Health Programs at ComForCare. Under the GUIDE framework, approved families receive care delivered in four-hour increments with personalized care plans coordinated directly with ComForCare locations, encompassing comprehensive daily living support from personal grooming to medication reminders, all delivered by dementia-trained caregivers through the company's established DementiaWise program.

ComForCare's participation leverages its existing infrastructure of more than 330 territories across the United States and Canada, providing immediate scalability for GUIDE services as the program expands nationwide over its eight-year timeline. This early adoption strategy positions the franchise system advantageously while demonstrating how established healthcare franchises can adapt quickly to evolving federal policies. The program's alternative payment methodology creates sustainable pathways for comprehensive care delivery while reducing overall healthcare costs through prevention of costly emergency interventions, establishing important precedents for how Medicare will approach chronic condition management in an aging population.
