Give the survey a Survey name for your own reference, then a Heading shown to customer and an Intro message — that is what the respondent sees at the top of the page.
The question types

| Paragraph | Free text. This is what the AI reads for sentiment. |
| Number | A quantity you want to average. |
Show in charts
Each question has a Show in charts switch. Ratings, scores, choices and numbers can be charted. Text answers are not charted — they are analysed for sentiment and themes instead, which the form tells you when you pick a text type.
Keep it to three or four questions

One rating question to drive the routing, one free-text question asking why, and at most a couple more. Every extra question loses respondents, and the rating plus the comment is where nearly all the value is.

Finishing up
Set a Thank-you message, and optionally a Redirect URL after submit. Note that review routing, if you switch it on, overrides the redirect for happy respondents.
Status
Draft, Active or Paused. Only an Active survey opens when somebody clicks its link — and only Active surveys can be picked in a marketing automation.
