An enquiry form is what a customer fills in. Everything downstream — routing, notifications, reporting — is configured on the form, so it is worth setting up properly once.
The basics

One form per job
Separate forms for genuinely different things — a general enquiry, a quote request, a careers enquiry. The form is a reporting dimension, so a single catch-all form means you can never tell which of your pages actually produces business.
